OTs conducting SDA assessments, therapists and care teams know better than anyone the needs of our participants. 

Environment plays a huge part in quality of life and we want to ensure that we do everything in our power to get the best possible outcomes for our participants.

Pathway to SDA gives you a place to direct your participants to so that they can be supported to achieve the type of housing and supports that you recommend in your SDA assessments.

We work closely with you to achieve the best possible designs for the builds and match with the best possible housemates.

We can’t do this without you! You are the experts – we are the facilitators.

Support Coordinators

Do you have participants on your case load that need a purpose built SDA home?

Do you need assistance to help them achieve the best outcomes for this goal?

SDA is a specialised area and support coordinators can’t be expected to be subject matter experts. 

We all want the best for our participants so it makes sense to engage the assistance of an SDA specialist to give your participants the opportunity to achieve the best outcomes for their SDA housing goals.

Pathway to SDA is an independent service that provides true choice and control for your participants and mitigates any conflict of interest for service providers. 

Pathway to SDA is ‘SDA support coordination’ only and does not replace you. In fact we work closely with you and your participants care team to ensure best outcomes. 

We understand that this is an emotional time for parents and the feelings that are associated. We work closely with parents and carers to reassure and support them every step of the way. 

Please note that Pathway to SDA is an unregistered provider so funding must be either plan managed or self managed. If your participants funding is agency managed, you can change it to plan / self managed by asking the NDIS planner to do a light touch plan review.

We are also more than happy to supply a quote that describes the services we provide to support your application for Specialist Support Coordination funding at plan review.
